隨著資訊產業的蓬勃發展與醫療科技的研發創新二者緊密結合,使得二十一世紀的醫學,經由高科技精密儀器的不斷推陳出新,對於疾病的預防和治療更具精確、有效的對策,為人類醫療史締造嶄新、輝煌的一頁。
即便如此,臨床診療仍不能端賴尖端醫療儀器,而應回歸到病人問診、理學檢查、檢驗判讀等全人醫療的基本功夫,並實際貫徹在醫學教育上,才能奠定臨床醫學的堅實基礎,健全醫療全方位的平衡發展。
爰此,本會王名譽理事長德宏教授,近日拜讀過韓良誠教授所撰「看Stanford 25有感」乙文後,頗有同感,特別再推介給全體會員分享,全文如下:
「看Stanford 25有感」

十月十五日(星期五),我前往成大醫學院林其和院長的辦公室,請教他一些醫學教育的事宜,當天他很高興地拿一本書名叫Educating Physicians: A Call for Reform of Medical School and Residency (由University of California, San Francisco School of Medicine的教授們Molly Cooke, David M. Irby, Bridget C. O'Brien三人共著)的書給我看,並且希望我在明年(2011)成大醫學系101級的白袍典禮上,講述「身體診察」(physical examination,請參見文末之註)的重要,特別是眼底鏡檢查,並且說,他已要求初步進入身體診察課程的學生要自己買眼底鏡,以利眼科教學。兩天之後的內科專科醫師口試當天(十月十七日),台大內科資深教授,王德宏前台灣內科醫學會會長,也很誠懇、熱心的告訴我說,他希望在台灣內科學界推廣「肛門手指檢查法」,並且提起留美的校友李中憲醫師,最近曾主動向王教授表示,希望「專程」返台推廣「肛門手指檢查法」。王教授也特別提到,他看完八月份的《景福醫訊》中,本人描述的「韓式肛門手指檢查教學法」相當的完整,希望我有機會做示範,並做成錄影帶,以利推廣。但是,當天我還是希望他邀請李中憲醫師校友比較有說服力,更何況又是他首先熱心地向王教授建議的。
無獨有偶地,又過了兩天,旅居美國的大女兒純芳,也寄了一則The New York Times十月十一日Denise Grady寫的“Physician Revives a Dying Art: The Physical”一文中提到“Stanford Initiative in Bedside Medicine”,“The Stanford 25”。另外,我也看了一些有關的文章如John Kugler和Abraham Verghese寫的“The Physical Exam and Other Forms of Fiction” (Journal of General Internal Medicine, Aug. 2010; 25, no. 8: 756-57)等等。
細讀這些文章之後,又再一次加深了我立刻寫本文的動機,因為過去多年以來,我一直希望以「高科技醫療時代的身體診察」為題,寫一篇自己的淺見,但是礙於才疏學淺以及有限的時間,一直不敢動筆,剛好Stanford 25及時給了我不少的鼓勵,也提供了很多我想寫的內容。自從今年2010年開始,世界第一流的Stanford內科部積極地想加強「床邊醫學」,醫師不但要具備內科知識,更要特別加強身體診察部分,使受訓者具備充分的能力,可以在床邊正確地做身體診察,如此則不但會建立更好的醫病關係,也可以挽救日益衰退的「床邊診斷」能力。
“Stanford 25: Fundamental, Technique-Dependent Physical Diagnosis Skills”是Stanford Medical School內科部Prof. Abraham Verghese和熱心推廣“physical exam skills and their diagnostic benefits”的一群醫師,努力完成的身體診察教學方法。為了方便讀者了解,我把其內容轉記如下:
- Funduscopic exam, papilledema etc. using panoptic and regular ophthalmoscopes
- Pupillary responses and relevant anatomy
- Thyroid exam technique
- Examination of neck veins/JVD for both level (volume) and common abnormal wave forms; tricuspid regurgitation (ventricularization of ‘v’ waves); canon ‘a’ waves, etc.
- Lung: Surface anatomy, percussion technique, finding upper border of liver dullness, finding Traube's space
- Evaluation of PMI, parasternal heave, and other precordial movements
- Examination of the liver
- Palpation, percussion of spleen
- Evaluation of common gait abnormalities
- The ankle jerk: if performed in a recumbent patient, one must have the right technique for each of the reflexes.
- Stigmata of liver disease from head to foot: be able to list, identify, and demonstrate
- Internal capsule stroke: list, identify, and demonstrate common physical findings: lower facial weakness, distal motor weakness, hyperreflexia, absent abdominal reflex on that side, abnormal plantar (Babinski) and abnormal tone, etc.
- Knee exam
- Cardiac second sounds: splitting, wide splitting and paradoxical spitting
- Evaluation of involuntary movements such as tremors, etc.
- The Hand in Diagnosis: recognize clubbing, cyanosis, other common nail and hand findings
- The Tongue in Diagnosis
- Shoulder exam (specifically testing rotator cuff tears, ac joint, etc.)
- Blood pressure assessment (this is more technique-driven than health care workers realize), Pulsus paradoxus assessment
- Cervical Lymph node assessment
- Ascites detection and abdominal Venous patterns
- Rectal Exam
- Evaluation of scrotal mass - differential between hydrocele, varicocele, spermatocele, testicular mass, etc.
- Cerebellar Testing
- Bedside Ultrasound
正如Prof. Verghese所說的,以上這些方法是用手就可以找出一個人的健康狀態的方法,並且說他的惡夢是,「明明一個病人在可以診斷、可以治療的情況下,因為我的手的技術不良而被錯失了」(韓註:應該是口、眼、手、鼻、耳、腦並用)。就以林其和院長希望我在明年七年級學生的宣誓典禮上強調的眼底鏡檢查來說,視網膜的變化以及眼底血管的情況、視神經的問題,這些所提供的線索,可以讓醫師好好地去推測全身可能存在的疾病,如心臟內膜炎、腦瘤、糖尿病、腎臟病以及愛滋病等等。至於王德宏教授正想大力推廣的「肛門手指直腸檢查」,醫師只靠手指就可以診斷手指可及,甚至不可及的大腸、直腸的癌症、攝護腺以及骨盤腔內的病變等等,這些都是Stanford 25的第1以及第22項所提到的。
Prof. Verghese認為目前的醫學院臨床診斷課程已不太重視身體診察的技術,以致於也許住院醫師雖然可以通過國考的內科筆試,但並不一定證明他「徒手」做身體診察的技術是可靠的。因為只知道理論並不一定可以正確地找出病因,更何況不管C.T.或是MRI都無法正確地告訴你器官的功能,運動神經有關的肌肉的收放,感覺神經的有無、強弱、半身不遂、顫抖、眼皮下垂以及有無失智,更不用說肚皮反彈痛……等等。另一方面,筆者自從忝為台灣內科醫學會的甄試委員之後,也深深地體會到,內科住院醫師的片段知識(fragmented knowledge)看來並不太差,但是在口試中問起身體診察的方法,或是要他們有系統、合乎邏輯也描述症狀或者疾病的病理生理學方面,則尚待加強之處甚多。
Prof. Verghese最擔心的是,在現代化的教學醫院中,很多的醫師花大部份在內科上班服務病人的時間,都坐在電腦銀幕前看病人的虛擬影像(“virtual construct of the patient, or the ‘iPatient’”),而不再認為「有血有肉」,躺在床上的病人才是更重要的。病人的照顧應該是全面性的,但是電子的記錄以及高科技的影像,不但使醫師遠離病床,也使床邊的服務不受重視,主治醫師甚至不做加護病房的「迴診」,轉為以遠距離儀器操控呼吸器以及藥物的使用。
此外,甚至也曾有學生告訴筆者,說有一位內科的總醫師告訴他:「自從升任總住院醫師以來八個月間,就不曾使用過聽診器了。」並且還說:「為病人做身體診察,只是浪費時間而已!」;有鑑於此,筆者常常告訴學生以及住院醫師,身體診察以及儀器的使用,兩者都不可偏廢,你一定要以病史以及身體診察先下診斷(make a diagnosis),並且只有在必要時,下一步才用適當的高科技設備來證實你的診斷(confirm your diagnosis),我都不會反對,這是高科技醫療時代的醫師應有的看病的態度,尤其是內科醫師,或者至少也可以說,病床邊熟練的病史詢問,以及身體診察,可以讓醫師減少不必要的檢查,換句話說,比較會「善用」現代醫學所可利用的高科技檢查,使之免於浪費,並且發揮該有的最大的功能。
Prof. Verghese說他觀察學生以及住院醫師,雖然他們強烈地希望在內科實習期間,把身體診察技術練好,但是他們在床邊所做的身體診察技術,已無法讓他們正確而可靠地把一些「signs」看出來,或誘導出來……。因此,筆者也有同感的是,在病例討論會時,病歷上所描述的cranial nerves intact, reflexes intact, no rales, no dullness, no splenomegaly, no ascitis, no hepatomegaly, no peritonial sign…很可能並不盡然,而且並不可靠,更何況是“S1 and S2 heard, no heart murmur”了。因此Prof. Verghese很謹慎,但也不客氣地說:「病例上所寫的應該是“… it is not a form of fiction, but represents an accurate observation. ”」,善哉斯言!因此他在談到Stanford 25的目標時提到,他的終極目標是:Junior resident可以對實習醫師教Stanford 25,而senior resident不但可以教,也應該可以示範,並且充分了解更近一步的與Standford 25有關的輔助性身體診察,這一點也正是王德宏教授這次向我談起的,希望台大內科此後應該努力的目標,以防止台大內科過去良好的一些傳統及特殊的技術,一點一點地流失,而沒有人可以好好傳授給下一代醫師。
最後,The Stanford 25 Team強調他們的終極目標是(為了讓讀者正確了解,以免曲解,我把這一段話的原文轉載於此):
“Create a culture (yes, it’s happening here) where we collect physical signs, alert others to patients with findings that are instructive, celebrate simple diagnoses that come from listening to a history well, and looking for and finding a critical physical finding.”
這一段話正如哈佛心臟科榮譽教授、諾貝爾和平獎得主Prof. Bernard Lown在他所著的The Lost Art of Healing: Practicing Compassion in Medicine上所寫的:“A British study showed that 75% of the information leading to a correct diagnosis comes from a detailed history, 10% from the physical examination, 5% from routine tests, 5% from all the costly invasive tests, in 5% no answer is forthcoming.”兩位大師級的內科老教授都有這樣的共識,難道我們不該反省自己?尤其是眼看在世界各國,包括台灣在內的許多國家,在C.T.的濫用上,已經到了令人髮指的地步,連醫學先進的美國,在C.T.的使用上也陸續有這方面的聲音出現,如Rebecca Smith-Bindman寫的“Is Computed Tomography Safe? The Uncritical Use of High-Tech Medical Imaging” (The New England Journal of Medicine, July 1, 2010; 363: 1-4)以及日文的「C.T.被曝大囯ニツポン
」,慶應大學的近籐誠醫師所寫的「C.T.檢查でがんになる」 (「C.T.會引發癌症」)……這一類的文章已多到讓我們也該嚴肅地去思考國內C.T.使用的現況了!如果我們同意諾貝爾獎得主、哈佛內科教授Prof. Bernard Lown在The Lost Art of Healing上的文章,診斷病人的疾病應該是「…5% from all the costly invasive tests…」的話,我們該不該反省?本文的最後,我想再次引用我在2009年六月一日「成大醫學院醫學系99級實習醫師宣誓典禮」致詞時的話,做為本文的結束:
「今天各位穿上了白袍,在開始第一天的臨床工作時,請牢牢記住,這一身的白袍,未必能穿上『名』,但也不可穿上『利』,而是要決心穿上一輩子的『責任』,以及對病人和家屬的『同理心』。也請各位每天在做完一天的工作,脫下白袍之際,要記得提醒自己:我今天是否無愧於心的,好好做到了醫學高科技無法做到的history taking和physical examination。更重要的是,要做高科技永遠無法做到的,對病人以及家屬的關心和安慰。」
註:陳定信教授在看完本文之後,傳真給我,說他唯一的建議是不要將“physical examination”譯為「理學檢查」,他說很久以前他曾經和林瑞祥教授討論過這一名詞,最後兩人的共識是「身體診察」或「身體診查」。另外他還談到:「我以前就覺得這個譯名有點奇怪,似乎“physical ”是「物理」而簡稱為理學,四、五年前Dr. Mark H. Swartz (他是Mount Sinai School of Medicine的教授,著有一本很不錯的Textbook of Physical Diagnosis)來訪台大,我請教他“physical exam”的“ physical”是與“chemical”相對的或是與“body”相對的意思,他告訴我應該是“body”。我想就像我們說體育是“physical exercise”一樣吧,因此建議“physical examination”應正名為『身體檢查』。林瑞祥教授以前曾建議或許可以譯成身體診察,似乎也是不錯的譯名。」實際上,多年以來,舍弟韓良俊醫師也非常反對將“physical examination”譯成「理學檢查」,他建議不妨譯為「身體基本檢查」。而我個人則因為先父以前看病的房間門外,所掛的牌子上就刻有「診察室」三個字。因此我心中一直在想,最好把“physical examination”譯成「身體診察」,就這一點上,也剛好和林、陳兩位教授的共識之一不謀而合。不知各位醫界先進有其他更好的建議否?
